Services: Behavioral Health Services

While we are best known for the quality of our traditional nursing and therapy services, the care provided at the VNA of WNY goes well beyond physical recovery. We appreciate that a person’s state of mind and cognitive wellbeing plays a significant role in their recovery.

The VNA of WNY has a team of RNs specialized in the care of individuals with a variety of behavioral health issues. Our priorities are to improve health outcomes, improve the experience of care for the patient, improve quality of life, and reduce health care costs. By coordinating care with other community resources and facilitating involvement with primary and behavioral health care providers, we aim to improve the experience of care and the overall health of this population.

Our behavioral health team can help care for patients with the following diagnoses including but not limited to:

  • Anxiety
  • Depression
  • Post Traumatic Stress Disorder (PTSD)
  • Bipolar Disorder
  • Borderline Personality Disorders
  • Schizophrenia Disorders

Treatment interventions vary depending on the patient but may include:

  • Conducting home visits
  • Providing education and support for the family members and patients regarding:
    • Medications, including administration techniques
    • Disease processes
    • Coping skills
    • Safe interventions that enhance the patient’s independence and safety in the home
  • Providing frequent assessment and rapid handling of medical and behavioral health issues which can prevent ER trips and hospitalizations
  • Administering regular injections, if necessary
  • Coordinating with your doctor every step of the way

Care Coordination:
Here at the VNA of WNY, we implement and monitor a holistic plan for each behavioral health patient. We help identify appropriate linkages, referrals, coordination and follow-up to needed services and supports.

Health Promotion Services:
We encourage and support healthy living concepts to motivate individuals to adopt healthy behaviors and promote better health management and wellness.

Referral to Community and Social Support Services:
We identify and provide referrals including but not limited to: medical and behavioral health care, entitlements and benefits, respite, housing, transportation, legal services, educational and employment, as well as many other specialized and supportive services.

Individual and Family Support Services:
We provide activities, materials, or services aimed to help reduce barriers to achieving goals, increase health literacy and self-efficacy skills as well as improve health outcomes of our patients.
